To make it simple, rolling a 1 makes you fail miserably on whatever the fuck it is you tried to do, say breathing, and rolling a 20 makes you succeed unforeseeably in anything, say riding a space unicorn into a black hole.

Rolling a 2-19, depending on what you're doing, (what obstacle is in your way that you're trying to overcome) you will have to roll above a certain number to succeed.
For example, you would have to roll above a 5, a 10, a 12, so on to succeed. You roll higher than that number, you pass. You roll lower than that number and you fail.
The number you have to roll is determined by the DM, or Dungeon Master, the Dungeon Master is the person who designs every challenge the players encounter. What dungeons they explore, what traps they find, what monsters they fight, and what loot they get. So yes, the DM does spend a lot of time on it.

As far as the DM part goes, it really depends on the type of campaign and DM you have, too. There's some more serious campaigners who go for more... scripted games, I guess? There'd be very drawn out details to go along with the story and the world. Then there's more casual DMs who will just make it up as they go along aside from knowing a handful of key points required to make the story work.

It's essentially like improvising a character to live through a book that you didn't write.

i always played the classic way, where you need say a 13 to pass a skill check, itll go from your roll + your skill level, and if its over 13 you pass and its successful, but if not you fail, and there are consequences depending on how hard you failed. you dont just succeed by the skin of your balls in some unpredictable way just because you rolled to try.

Natural 1 means the die landed on a 1.
Natural 20 means the die landed on a 20.
If you rolled an 18 and they have a 2 point bonus, that would be a 20, but that would not be a natural 20.
If you rolled a 3, but had a 2 point penalty, then that would be a 1, but not a natural 1.
"Natural" refers to the number rolled. Rolling a 1 is a critical failure and special things happen that are usually bad and painful. Rolling a 20 is a critical success and special things happen that are usually awesome.
