The amount of concentration spent actually working with an operation, especially transplants, is much higher than just staying awake. You would be exhausted.

I commended the doctor for going to all that effort for only a few more years, since he looks pretty ******* old in the first picture.

After more than 3 hours there is the possibility of permanent brain damage while under anesthesia. The likely hood increases every additional hour under anesthesia. If connected to a heart bypass machine during surgery, the likely hood of impaired motor function and decreased hand-eye coordination increases exponentially for every hour the heart is bypassed. The reasons for these phenomenon are not yet understood.
