In Europe, they were built on/around/in churches because though the wealthy and the fortunate kept dying through the ages, the number of churches with "hollowed ground" or earth gathered from Holy sites in Israel stayed small. As time passed, churches ran out of places to bury the bones, and let them build up. Eventually, they hired an interior decorator or something to figure out what to do with the bones.   
Ossuary/charnel houses that I know of off the top of my head are Sedlec Ossuary (Sedlec, Czech Republic) and Capela dos Ossos (Évora, Portugal).
