The Runaway Bride featured the character of Donna before she became the companion
Clara is a complicated case we don't understand yet but that Clara (and the one from the Snowmen) is possibly the same one. In any case it's still part of the story arc

From older series Ian Marter played a character in Carnival of Monsters a few years before he joined as Harry and Colin Baker appeared in Arc of Infinity the year before he became the Doctor.

Any and all instances of actors playing two different roles were rendered irrelevant INSTANTLY back in the 80's when Colin Baker [The Sixth Doctor] played a Time Lord commander alongside the Fifth Doctor.

No. The first mention of the Last Great Time War is in the 2005 series. No mention of it had ever been in any TV episode, comic, novel, audio, or anything.
It is assumed it occured at the end of the 8th Doctor's life, although it's never been stated.
