when the guy ejects at the end he almost certainly died

there is a minimum distance you can eject from bellow that distance you are not fired up high enough for your chute to open in time

Nope, a part of the ejector seat's purpose is so that it can be fired in case of emergencies during take-off or landing keeping the pilot alive.

There is a minimum altitude that an ejector seat can be fired at whilst in inverted flight which varies from seat to seat.

Yes, I looked at that case as a part of my degree in Aerospace Engineering, the parachute had a fault in its deployment system which stopped it from deploying properly - without any fault, it would have opened.
