If you watch the show, Captain Jack tells the doctor he's the Face of Boe almost directly
As he walks away from Tennant, he talks about his inability to die, then recalls he used to be a model for a calendar, and said "What did they call me?...Oh ya, the Face of Boe or something like that."

He said he was the poster child for the Boemian Penisula. But this makes since. When he says he is the face of Boe, that is the reveal, but when you really think of it, the show neer decribes why he is just a head as the face of Boe. I subscribe to this theory.

Buut if you watch torchwood, there's that one where Harkness was blown to smithereens, and then he slowly grew his whole self back. So would he not just grow his head / body back when detatched?

Perhaps someone close to Jack (not realizing that he would not die) put him in a vat of juices that would stabilize his head so that he wouldn't die immediately, if at all. Then, Jack is alive and his inability to not die might not regrow his body because he didn't actually die, but his simply headless. Then, over millions of years his head simply heals inside the tank and ages to the point of being the Face of Boe.

Or maybe the vortex manipulator isn't Jack's, but the other Time Agent from Torchwood.

but why did the Face of Boe die if Jack is immortal?

good question, me. Well you see, when Jack became immortal, it was because Rose (the God-like Rose, after looking into the heart of the Tardis) wanted to never see him die. So she made him un-killable (note: not immortal). Jack can age, but incredibly slowly.
But why did he die when he did?
The furthest forward in time that Rose went was to New Earth. To Rose, that was the end of her time. So the FoB died just outside of Rose's life, so that she would never have to see him die.

I'm pretty sure the explanation was that Rose basically put life energy into him. An extreme amount to the point where he appeared immortal. The time energy was not limitless and eventually, he would still die just like any other living thing. Sort of like the doctor except without the regeneration part. So basically rose just gave him energy one would need to live countless years.

I like your explanation more, though.
