I've never seen a back up brake pedal before, unless you count the emergency brake which is usually in a much different position. It is still strange that there is no gear shift for park, neutral, reverse, and drive.

Maybe the car is old, and it has its gear shifter installed into the side of the steering wheel.
Not many, but a few older models had it that way. We simply do not see it, because of the perspective. But I do not know if they were popular in the USA or not.

my first car, a 70s mercedes, had a third pedal: the e-brake, which you released with a knob on the dash. some newer trucks still have this system. doesn't quite look like a big pedal like that but its still a third pedal.
