Luna do not posses the power to communicate trough the moon in the real moon.
This brings to the only conclusion that Scootaloo is dreaming and everything that happened after she was suppose to fall down is a dream. She's either in a coma or dead and did not get saved by Rainbow Dash in the nick of time.

Also, how do we know Luna can't communicate using the real moon? She can already invade your mind while you sleep, how hard would it be to reshape the shadows on the moon?

What's so noticeable is that Luna did appear on the moon just before she destroyed the headless horse in the dream then pretty much the exact same way after Rainbow Dash saved Scoot in what you suppose, the real world.
Then when she'd communicated with Scootaloo only Scoot could hear Luna's voice when she'd "spoke" from the moon. So we can conclude she did communicate on a psychical level in addition with reshaping the shadows on the moon not mention to synchronize it. Notable mention is that shadows are not a power of the night, but from the day since shadows are but blocked light.
And to back up my argument additionally, before Scootaloo left the cave she though she'd heard the sound of the headless horse which actually was Rainbow Dash snoring in her heavy sleep with those ear-plop thingy's making highly unlikely that she would even notice Scoot leaving the cave in the first place.

Luns possess the power to raise a celestial object the size of.... well, the moon, and possibly to move the very stars.

Communicating through the moon cannot be too much of a challenge.

RD might have woken up to pee or something and noticed she was missing. And sure, it could all be just a coma dream. But would you want that?

I think you have never woken up for the need to pee within the hour you fell asleep since your body goes into the most heavy sleep during the first hour making it most unlikely to wake up for such a simple need as pee. Either way, you sleep as most heavy during the first hours and with ear thingy's there's most likely that RD wouldn't wake up unless her mane was on fire.
Do I want her in coma? Well of course not. I didn't want her to die either but still Final Dreams of a Filly is almost the best fanfic I've ever read. So what I want isn't really relevant.

Equine brains don't function quite the same as humans. For example: Graffias get about 30 minuets of sleep a day in five 6 minuet cycles. For all we know cartoon ponies sleep cycles could be 5 minuets long rather than the 90 minutes of humans. Plus, scoots did scream rather loudly so if any of them had woken up, they all would have gone out to search for her. RD, being the only one that could fly, found her first.

Well it's known the ponies got a 24hr day and they do indeed sleep trough the entire night, which caused Luna to go berserk. So it's pretty safe to assume that ponies sleep cycle are about the same as a humans.
When Scoot went hysterical she did indeed scream rather loudly, however if somepony would wake you up with a bunch of screaming you'd tell her to shut the buck up and after the night adventure the others seemed unaware of what had happened.

Just because the sleep 8 to 10 hours a night doesn't mean their sleep cycles are 90 minutes long. They might not even have sleep cycles and are just either asleep or awake. I think most insects work like this but my memory's fuzzy on that. She did leave the cave rather rapidly when she screamed so it's very possible that they didn't have time to yell at her after being awoken.
