The shape just makes it very inefficient, since the headers and manifold would be much larger than an inline engine. Takes up more space than necessary.

Not true at all due to the shape of the radial you could package it almost anywhere.

It's the reason they're used in prop planes and not cars. The engine bay would be quite tall, increasing the drag and reducing performance and fuel economy.
