That's easy. The examples would be:

Normal violence - a fist fight with real consequences, like bloody noses, bruises, and being sent to the hospital

Fantasy violence - fighters jumping 50ft in the air, destroying buildings with one hit, getting stabbed and not dying, etc.

basically violence centered around a concept not present in current or recorded history.
so a sword fight could be realistic violence, but a sword fight with zombie abe lincoln would be fantasy violence.
trust me when i say the actual law is about as specific as that, at least in the US, which is how a lot of shows make TV standards despite seriously graphic content because its technically classified as "fantasy" and as such undergoes less scrutiny by the people who actually rate things.

MGSV was going to be AO, but no stores would carry an AO game, so they had to dumb it down to M.

Yes, actually, it's the same reason Fallout 3 had to remove child killing, even after being able to do it in Fallout 2 when the ESRB was just starting and so was much more relaxed.

I personally think we need to either
a) reformat the warning system
b) get companies to label things properly

Never been offended or anything its just well, T seems to have become very rare as a rating whilst, everything is rated M despite a huge disparity in content.

For example a comparison of games I did last year
GTA 5, M, ESRB descriptions Blood and Gore, Intense Violence, Mature Humor, Nudity, Strong Language, Strong Sexual Content, Use of Drugs and Alcohol
Cod Advanced Warfare, M, ESRB descriptions : Blood and Gore, Drug Reference, Intense Violence, Strong Language

No problem with either game, but having them both in the same category despite one being clearly more "mature" than the other seems off to me. It's like the video game equivalent of lumping the Walking Dead and Game of Thrones in the same category. inb4 it turns out they are because i'm just assuming

I became interested in this because I used to help my parents and subsequently a couple neighbors understand the content that was present in these games they were considering purchasing for my siblings / their kids. I never told them if it was appropriate or not I just made sure they understood what was present in the games and left the decision making to their discretion. because it seems many less than tech savvy parents just ignore M ratings now because many M games like COD really don't have much in them despite the rating

Games now a days are actually going down in rating and TV shows that show more explicit things are going to a lower standard as well. Like mature shows are being pushed more towards teens, and again, movies are doing the same thing. As those games are concerned they're both violent games which warrant a M rating.

its not so much that, most game stores just dont carry ao games, so the game would just sell like **** . not worth the investment. you may see some low budget titles with an ao rating. sort of like the nc-17 rating.
