Yeah, I suppose that's a bit of a spoiler, but you were kind of asking for it.

If you really want to know, but be warned because this really does contain spoilers:
District 13 was supposedly wiped out by the Capitol in a nuclear/firebomb strike after they engaged in a power struggle. They pretty much moved underground in a military-esque society. They are in charge of nukes and stuff, so the Capitol has made a peace treaty with them. Not quite accurate, but that's the general gist of it.

They tell you a vague description of happens to district 13 in the 2nd movie. Also the 3rd movie comes out this year and the last movie comes out next year. From what i've been told ~November or some month around that is somewhat known as "twilight time" or something similar. I'm getting the impression that hollywood will now use that time to release book movies. I curious to see what book they'll do next, but that's not until 2016.

I'm sorry, but I'm pretty sure that each District is just a walled city. Giving large territories is meaningless, because all that matters is that city and a small area nearby. Hence why District 12 was destoryed so easily, it's one city. . Infact, you probably would not to be able to see any District on a map of North America at that scale due to the Districts being so small.

Basically, the setting is post-apocalyptic. It's unknown what exactly caused the demise of the world, but all that is known is that large land masses changed shape due to the rise of sea level and lots of people died.
